View Details About The Awards
Step 1: Download and complete an Awards Nomination Form
- Right click on the Download link above and "Save target as" to
save to your desktop
- Open the form using Microsoft Word and fill out all applicable
fields.
- You MUST complete one form for each applicant.
Step 2: Save each Nomination Form
- You MUST complete one form for each applicant.
- Nominations CANNOT exceed 3 pages total. Any nominations exceeding
three pages in length will be disqualified for consideration.
- Once you have completed all of the form fields, save the file using
the following file names:
award_nomineelastname_date.doc
Example: hughesaward_smith_2008march1.doc
Step
3: Upload Award Nominations to the IMAPS server
- Use the Upload Tool below to transfer your nominations to the
IMAPS Server
- You may upload up to four nominations at a time
- MAKE SURE each file is named using the example format above
- Use the browse button to locate each file on your computer
